To determine eligibility for health benefits, refer to the following information and copy of article 10 of the contract.

Notifications of eligibility are sent out via campus mail at the third or fourth week of the current semester.

Upon receipt of eligibility notification, complete one
(Only 1) of the following Kaiser Health or delta dental or delta care or vision form.

To expedite the process, employee benefits is now processing enrollments on line.

Kaiser and delta will mail I.D. cards to home addresses.

In case of an emergency at the dentist, visit Delta website and print your own I.D. card.

Fall eligibility is effective October 1st through March 1st and spring eligibility from march 1st through October 1st.
