ACADEMIC INTEGRITY & CONDUCT NOTICE

As part of our commitment to students partaking of services offered by the Assessment Center, we are committed to provide an academic environment conducive to academic honesty. Thus, students are expected they will behave in a fair and responsible manner. To this end, as part of our check-in process, students must certify to the following statements before they can assess.

As testament to my commitment and readiness to join the Santa Monica College academic community, and as a condition
for testing, I hereby affirm that I will abide by the SMC Honor Code, which reads:
In the pursuit of the high ideals and rigorous standards of academic life, I commit myself to respect and uphold the
Santa Monica College Honor Code, Code of Academic Conduct, and Student Conduct Code. I will conduct myself honorably as a responsible member of the SMC community in all endeavors I pursue.

Declaration:
I ________________________________________________, willfully declare that in the process of taking the above test,
I will abide by the SMC Honor Code by not engaging in cheating, fabrication or falsification of information, plagiarism,
abuse of academic materials, complicity in academic dishonesty, disruptive behavior, willful disobedience, habitual profanity, impersonation, etc. I understand that engaging in any of these (or other disruptive) activities will result in an immediate referral to the Office of Student Judicial Affairs for disciplinary action. Additionally, willful disregard for assessment protocol and/or instructions provided (oral, printed, or on the screen) will result in forfeiture of my test results and inability to retest for a minimum period of three months. I understand I will be held responsible for any retesting fees. Willfully switching over to, or initializing a new test without authorization will result in a charge of $10 per test and will prevent me from retesting for a minimum of three months. Any of the above actions will result in an assessment hold on my records, which in turn will trigger an enrollment block. If I am not a SMC student, violations will be reported to my institution of origin.
