Please complete all requested information. Once we receive the form and verify you are eligible for remote testing, you will automatically receive an email from HVService as the sender. This email will provide you with the required registration number, along with some other information. Your registration number is valid for 45 days. WE WILL NOT ISSUE A NEW REGISTRATION NUMBER. Please be sure to complete your test within the time allotted.

I understand that in order to process this request all information above has been provided. I further understand that I am solely responsible for any proctoring fees incurred as stipulated by the Testing Center above. Further, it is my responsibility to ensure that I complete the COMPASS remote test within the period noted in the notification email I will receive upon submitting this form. The Santa Monica College Assessment Center will not re-issue a new registration number to me if I fail to complete the test. I further agree that I will adhere to the SMC Assessment Center ACADEMIC INTEGRITY & CONDUCT NOTICE. Failure to adhere to it will result in forfeiture of test results, retesting fee, disciplinary charges, etc.
