The following forms should be submitted to:

- AB540 Affidavit Form (Non-Resident Tuition Waiver) Your official High School transcript (including graduation date) must be submitted along with this completed form.
- Pass/No Pass Petition (formerly Credit/No Credit) Please note the submissions deadlines as they are strictly enforced! Deadlines to submit the Pass/No Pass petition: Friday of the 4th week for Spring and Fall semesters. Friday of the 2nd week for ALL short term courses or Winter and Summer sessions. Friday of the 1st week for all classes that are less than 6 weeks in length. You MUST confirm receipt and approval of your form.
- CSU General Education Certification Petition There is a $5.00 non refundable fee (make checks payable to "Santa Monica College") for processing this request.
Spring filing dates: January 1- July 31 Fall filing dates: October 1- December 1 An official SMC transcript will be sent to the campus indicated along with your CSU GE Certification. If deficiencies are not resolved within one semester/session, you will need to re-apply for certification and re-pay the $5 processing fee. It takes approximately six weeks to process the petition.
- Dual Enrollment Consent Form (This is where SMC college classes are taught off campus in high schools)
- Duplicate Diploma/Certificate Request Form
- Graduation Petition (For more information about Graduation, go to the Graduation web page)
Spring filing dates: Start of Spring semester through April 30 Summer filing dates: Start of Summer term through July 31 Fall filing dates: Start of Fall semester through December 1 It takes approximately six weeks to process the petition.
- High School Concurrent Enrollment Approval Form
- Intersegmental General Education Transfer Curriculum (IGETC) Petition There is a $5.00 non refundable fee (make checks payable to "Santa Monica College") for processing this request.
Spring filing dates: January 1- July 31 Fall filing dates: October 1- December 1 It takes approximately six weeks to process the petition.
- Petition for Special Consideration Complete this form and deliver, mail or fax it to the Admissions office. This form will be reviewed by a committee. Generally, students receive a response within 3-4 weeks.
- Online Petition for Special Consideration Alternatively, you can fill out the online form and get a response via email. All petition submissions will be reviewed by a committee. Generally, students receive a response within 3-4 weeks.
- Petition for Late Withdrawal This form may only be used to request a withdrawal from a course when the student is earning a C or better in the class and an extenuating circumstance has occurred. Submit the form to the Admissions office with proper documentation and the required instructor approval by the appropriate deadlines which are noted on the back of the form.
- Request to Waive High School Transcript Requirement
- Study Abroad Petition Verification Form This form is to be filled out BY THE INSTRUCTOR upon return from the Study Abroad experience.

- Enrollment Verification online request form: Please note that verifications made online through "Degree Check" are designed primarily to meet the needs of third-party inquirers. In general students who need verification of their enrollment have a specific need for an official signature and school seal. That can only be obtained by going to the SMC Admissions office in person. From the Degree Check system, the student will only receive an email confirmation of their enrollment which is similar to the information that they can print themselves from the Student Self-service system.

The student must bring this form, completed and signed to the Admissions office.
