Faxing Test Results to Other Colleges

Requests for faxing SMC placement test results to other colleges must be submitted in writing in person to the Assessment Center. You must provide us with the appropriate contact information for that college: fax number, contact person, telephone number, etc. Every attempt will be made to fill your request within one business day. However, there may be times when the Assessment Center will be very busy and it may take longer.

Test Results Faxing Service

The SMC Assessment Center will gladly fax your test results to the college of your choice. Please follow the instructions below to request this service. We will do our best to fill your request within two business days.

Instructions:

• Download and complete the Request & Authorization to Fax Official SMC Assessment Placement Results form;
  
   
• Fill out all of the information requested, including the college name, address, contact information, etc. It is crucial you sign and date the request. Incomplete forms will not be processed.
  
   
• Cost: Test results will be faxed for FREE to any California Community College located south of Santa Barbara. For all other colleges, your request must be accompanied by a $3 processing fee (please make your check or money order payable to Santa Monica College—cash and credit cards are not accepted).
  
   
• Requests must be submitted in person or by mail if we will be faxing your test results to a non-California community college or to any California community college located outside the southern California area. Otherwise, you may submit your request by fax.
