Complete a minimum of 100 hours volunteering 4 hours a week at UCLA Santa Monica Medical
Center. Rotations include serving as a greeter at entrances, providing patient transportation,
and/or assisting as a clerical floater between departments.

Requirements
At least 16 years old
Currently enrolled in college or hold an accredited degree
Non UCLA matriculated students studying on an F-1 visa are not able to apply
Commit 100 hours (4 hours a week for one year within your assigned hospital rotation
Proof of varicella, MMR and QuantiFERON-TB Gold blood test
Valid, government-issued photo ID (driver's license, state ID, passport, or military
ID)
Attend a mandatory information session
Online application and 500 word essay, stating the applicant's reason for volunteering
Required forms, documents and proof of immunizations
Background check for anyone 18 and older
