Read the instructions on verification form carefully prior to attending classes. One form is required for each class you intend to receive certification.
It is your responsibility to follow instructions listed on the form and make sure your instructors initial the form every time you attend class.
It is a government requirement that you will only be given the hours that you actually attend the class--so if you arrive late, leave early, leave during breaks and not return to the class, you will not be given credit for that class, even if you are enrolled.
It is your responsibility to submit the forms to our office, not your instructor's responsibility.
Note that we now require the forms be emailed. They should be emailed to our main administrative email account at commed@smc.edu.
To email: you will either need to scan or photograph the completed, signed form. If photographed it should be a clean, high-resolution image against a neutral background. Each form should be a separate scan or photo image file.
