Submitting Transfer Application Documents for Completion
Admissions & Records can assist in completing forms required for your transfer applications such as the College Report, Transfer College Report, Dean's Report, and other similar, or related, documents. Please follow the below instructions for submission, requests are responded to in the order they are received and can take up to 5 business days to process. Be sure to check your application portal so that you are aware of any deadlines, and submit your forms on time to avoid issues with your application.
- Download the document as a PDF file from your application portal - please check with the transfer school directly if you are unsure about what they need from you.
- Complete any required "Applicant" or "Student Section" portions of the document, and sign in any applicant signature areas - do not upload a blank document if there are portions of the form intended for the student to complete. You will be asked to correct and resubmit the request, and it will delay the processing.
- Submit the document to us online using the Enrollment Verification Request Form (You will be required to sign in to your SMC network account to access the form).
- When completing the request form, select the menu option for "Transfer Document", and then upload and attach your document to the request form.
- Select the Email option for Method of Delivery, and then provide the University email addresses that you would like the completed document(s) sent to. You can include your own email if you want a copy of the document for your records. Most schools will want to receive the completed document directly from a College official and not the student, be sure to check with your transfer schools directly if you have any questions about their policies.

Submit the form for processing - you will receive an automated emails when the form is submitted, and processed. Be sure to check your student email account to stay up to date on the status of your request.
Requests typically take 3-5 business days for processing. Be sure that you are aware of your application deadlines, and submit your request early, as there is no option for expedited service.
Midterm Reports: The Midterm Report asks the applicant to provide their current grades for any in-progress coursework, and does not require any input from a College official. Please contact your course instructors with any questions regarding your current grades as Admissions & Records does not have access to this information. If you would like us to send your Midterm Report to your transfer schools, please be sure to fill it the document out completely, and submit it using the Enrollment Verification Request Formusing the above instructions.
