• Participation in Disabled Student Programs and Services (DSPS) shall be entirely voluntary.
• Receiving support services or instruction authorized through DSPS shall not preclude participation in any other course, program, or activity offered by the college.
• Personal information will be kept confidential in order to protect against unauthorized disclosure. Portions may be shared with the Chancellor’s Office of the California Community Colleges or other state or federal agencies; however, disclosure to these parties is made in strict accordance with applicable statutes regarding confidentiality.
• Student will provide DSP&S with the necessary information, documentation and/or forms to verify disability-related functional limitations.
• Student will utilize DSP&S services in a responsible manner. DSP&S has service provision policies and procedures, which must be adhered to for continuation of services (e.g. sign language interpreting, note taking, test accommodations).
• Student will comply with the SMC Student Conduct Code and all other applicable laws and regulations related to student conduct, as detailed in the college catalogue.
• Student will make measurable progress toward the goals established in their Student Educational Contract, or, when enrolled in a regular college course, will meet academic standards established by the college.
• Student must meet with a DSPS counselor/faculty specialist at least once each academic year or, preferably, each term of enrollment to plan their Student Ed. Contract.
• Students are required to sign a document agreeing to the above Student Rights and Responsibilities. If they do not comply with these rights and responsibilities, they will be noticed in writing of pending suspension of services. Decisions relating to issues pertaining to the Student Conduct Code may be appealed by contacting the Disciplinarian at 310/434-4655. For issues concerning academic adjustments, appeals can be made by contacting the ADA 504 Compliance Officer at 310/434-4442.
