Forms for F-1 International Students

  Check-In Form
New Students
must complete the Check-In Form within 10 business days of arrival in the United States. 
Transfer Students must complete the Check-In Form after their new Student Seminar.     

CSN Memorandum
Current students who are sponsored by CSN who need to verify the number of units enrolled at SMC. Students must enrolled in their classes before submitting this form. The form must be completed and uploaded to intlcounselor@smc.edu.

Emergency Ten Day Refund Request Form
Current students who had to withdraw from SMC and depart the U.S. after fees were paid. Must submit with a copy of airplane ticket as proof of departure.

Health Insurance Refund Request Form
Current students who had to withdraw from SMC after health insurance fees were paid. Must submit with required document (listed on the form).

I-20 Extension Request Form
Current students whose I-20 will soon expire but wish to continue studying at Santa Monica College. Must submit form no less than 30 days before the end date on your I-20 with all required documents listed on the form.

I-20 Signature/Replacement Request Form
Current students who are traveling, need an I-20 replacement, changed major or personal information, or have a child and/or spouse coming to the U.S. on an F-2 visa. Submit I-20 travel signature requests at least 2 weeks before your departure date. May require additional documentation to be submitted - see form for details. (SMC Login Required)

Medical RCL Application Form
Current students who have a serious, temporary illness or injury and a recommendation from a medical doctor(MD), doctor of osteopathy (DO), or licensed clinical psychologist to be below full-time enrollment status for the upcoming or current term.

Medical reduced course loads are very restricted. Please read the instructions and guidelines located on the form very carefully before submitting a Medical RCL Application. (SMC Login Required)

OPT Employment and Address Report Form
Current students on Post-Completion OPT who need to update their employment information and/or U.S. home address or terminate their OPT period

Submit to the IEC in person or by email. F-1 rules state that employment and address information must be up-to-date at all times. Unreported employment is counted as unemployment. Exceeding 90 days of unemployment is a status violation. (SMC Login Required)

Reinstatement Assistance Request Form
Current students who are out of status and had their I-20 terminated who wish to return to F-1 status and continue at SMC.

Reinstatement requests will be reviewed by the Reinstatement Committee (allow two weeks for review). You will be notified via your SMC email. (SMC Login Required)

SACM Online/Hybrid Course Verification Form
Current students who are sponsored by SACM who need a letter to verify the number of online units completed at SMC

The form must be completed and submitted with the SMC Verification Letter Request form. There's a $3 fee for each letter/copy requested. Pay fee at the Cashier's Office after receiving payment form from IEC.

Transfer Out Request Form
Current students who are transferring to a new school. Must submit the online completed form and upload an official admissions letter from the new school. Processing times are up to 10 business days.

 If you are transferring at the end of the Fall 2023 semester you can submit this form starting on or after December 1st, 2023.  (SMC Login Required)

Verification Letter Request Form
Current students who need a letter to verify their status, fees, units, or other requested verifications (e.g., CSN Study Assurance forms). The form must be completed and signed. There's a $3 fee for each letter/copy requested. Pay fee at the Cashier's Office after receiving payment form from IEC. (Fee is currently waived at this time). (SMC Login Required)

Withdrawal Form
Current Students who will no longer study as an F-1 student in the U.S. Reasons include terminating or completing studies to depart the U.S. or approval for a change of status. Complete and submit to the IEC in person or by email. Submitting this document means your I-20 will be terminated or completed. (SMC Login Required)
